Call the Midwife season 8 episode 2 will arrive on BBC One on Sunday, and we’ve got another new patient for Lucille at the center of it.

In the sneak peek below, you have a chance to meet Ms. Milgrove, an older woman who Lucille is trying to look after — even if she does her best to refuse treatment, in turn. Milgrove is a hoarder, and because of that she’s surrounded by belongings and seems to be fairly detached from the rest of the outside world. We also wonder how much her behavior is caused as a result of depression, as she doesn’t seem to be really invested in her own life, let alone the rest of the outside world.

Yet, this sneak peek is a perfect representation of what the women of Nonnatus House try to do more so than anything else: They care for other people and want to ensure that they have the tools in order to have a proper future. It’s not a particularly easy thing for them to do sometimes, especially when they have difficult patients like Ms. Milgrove, but that isn’t going to stop them. Lucille has a full year at her profession at this point in Poplar alone, so she’s accustomed to being able to handle these sort of difficult circumstances. The first order of business here is really earning Milgrove’s trust, and then after that, she may be able to better treat her. A part of Milgrove’s resistance to any treatment or help here is likely coming out of fear — she’s used to a very specific way of things when it comes to her life and you have to imagine that it is going to be rather difficult to deviate from that — at least at first.

By the end of this episode, we’re crossing our fingers for some sort of happier resolution — though we know on the way to getting there that there are very few guarantees. Call the Midwife is a show that does often focus on reality — and sometimes, patients do have untimely and tragic ends.
