He was a snake in the original story but it seems that it was so much easier to hold a secret over someone’s head in the Victorian era. I think it’s safe to say that the real drama here is likely to involve Mary Morstan. I don’t want to give away anything from the Empty Hearse but she said something and knew something in that episode that I was surprised someone like her would know and it was very important to an element of the story. John met her at work and he only knew her a short time before he proposed. That’s not a spoiler it’s from the canon. So my guess is she’s got some back story or her family does and that’s what will play out.
